Registered owners that have received a Penalty Order under the Automated Speed Enforcement (ASE) Program may request a review of their Penalty Order by a Hearing Officer within 30 days of receipt of their Notice of Decision from the Screening Officer.  

If you, as the registered owner of the vehicle, have lost your Penalty Order, or would like to obtain another copy of your Penalty Order and/or the Certified Statement of the Provincial Offences Officer prior to your Hearing, you are required to complete and submit this form, at minimum, seven days prior to your scheduled Hearing.

Documents will not be provided until your scheduled Hearing is confirmed.

Fine responsbility

I am aware that if there is a fine to be paid after the Screening or Hearing appearance, the ultimate responsibility to pay the fine(s) rests with the Registered Owner of the vehicle, in accordance with Section 5 of Ontario Regulation 355/22 made under the Highway Traffic Act.
